Information for First-Time Patients

Welcome to Velocity Physiotherapy. The information below will help to make your first visit with us a pleasant and positive experience.

• What to expect

  Please arrive 15 minutes early to your first appointment for patient registration. Plan for your first appointment to take up to 75 minutes; the initial evaluation will take 60 minutes and additional administrative issues can take up to 15 minutes. All subsequent visits will last 60 minutes.

• Insurance coverage and payable services

  To determine your coverage and benefits for physical therapy, you must contact your medical insurance company. Coverage depends upon your medical provider and the specific plan you have chosen.
  Please use our Insurance Benefits Worksheet to help determine your healthcare coverage and benefits when speaking to your insurance company’s customer service representative. You can download the worksheet by clicking here.

  NOTE: It is your responsibility to understand your insurance eligibility and benefits from your insurance company PRIOR to your first visit.

• New Patient Paperwork

  In advance, please download the New Patient Registration Packet and bring the completed forms to your first appointment. You can download the packet by clicking here.

  Please be sure to bring the following items with you to your first appointment:

  • New patient registration forms.
  • Comfortable clothing. For sports related treatment please bring shorts, t-shirt and sneakers.
  • Referring provider's updated, written prescription, referral, or pre-authorization.
  • Current insurance card.
  • A method of payment (checkbook, credit card, etc.).

Patient FAQ's

• Is parking provided?

Yes, free 2- hour parking is available in spots marked "visitor" in either of the two building parking lots facing 34th and 35th streets. Street parking is also available on 34th street

• What can I expect at my first appointment?

The initial appointment will include an evaluation as well as physical therapy treatment. Please arrive at Velocity Physiotherapy 15 minutes early. It is recommended that you wear loose clothing for your appointment (e.g., shorts, t-shirt and sneakers).

• What will my insurance cover?

It depends on your health insurance coverage benefits. Please call your insurance provider to verify your benefits. If you require further assistance regarding insurance coverage please call Susy at (877) 461-9918or email billing@velocityphysio.com.

• Will I need a referral for physical therapy?

In most cases, your insurance coverage will require you to obtain a current written referral for physical therapy. Please call your insurance provider to verify whether a written referral is required.

• If I need to cancel, how much notice do you need?

  We ask for 24 hours notice on all cancellations. Appointments not kept or cancelled within 24 hours of the scheduled appointment time will be charged a $50.00 cancellation fee.
